We understand that each person has their own needs and will require different levels of care and support to assist in working towards specific goals and outcomes. Houses of Hope Supported Living Services operates and is geared towards working with the service user, using a thorough, assessment, care planning and review system to highlight the areas for working with the service user and specifically the key targets or priorities.

To ensure we are providing the best possible care at all times and meeting the service users needs, care plans are regularly reviewed as the placement progresses to reflect any progression or digression made by the service user during their placement.
- Tailored care plans help to assist and map out the service users needs
- Plans should be discussed, reviewed and modified if need be to complement the service user.
